Semaglutide vs. Tirzepatide: the iPhone 7 vs. the iPhone 15

Most of the horror stories you've heard ("I couldn't keep anything down," "I couldn't stay on it") are almost always semaglutide — the older, GLP-1-only medication. Caroline doesn't recommend it to most women. Tirzepatide adds a second mechanism (it's both a GLP-1 and a GIP agonist), and clinically, that means significantly fewer GI side effects and better results. It's her recommendation across the board.

So, Do I Have to Stay on This Forever?

It's the question Caroline gets asked more than any other. Microdosing is flexible — start and stop as you like. For weight loss, if you've genuinely struggled with your weight most of your adult life, she treats it like any other chronic condition you manage: stop the treatment, and you stop the benefit, the same way stopping blood pressure medication or stopping the gym does. But if your weight gain is tied to something specific — hormones, an injury, a hard season of life — she can often taper you off strategically, stepping down at least as slowly as you stepped up, alongside the habits (protein, water, resistance training) that make the results stick.

Two Myths, Debunked

Thyroid cancer: the black-box warning comes from a specific rat study, involving a type of thyroid tissue expression rats have that humans don't. It has never occurred in a human on these medications.

Pancreatitis: this risk is tied to losing weight too quickly, which is a dose-related issue — it's exactly why dosing is managed by your prescribing doctor rather than self-adjusted.

Where It Actually Comes From

Why the Pharmacy Behind Your Peptide Matters

The Question Everyone's Asking Lately

On Retatrutide (the New "Triple Agonist")

Retatrutide adds a third mechanism on top of what tirzepatide already does — it's also a glucagon agonist, not just GLP-1/GIP. Right now it's only available through clinical trials (still in phase 3) or research-grade/gray-market sources, and EllieMD isn't carrying it while that's true. The weight loss results are significant, but that third mechanism has been linked to arrhythmias in trials — which is why Caroline doesn't consider it a good long-term, maintenance option. Tirzepatide remains her long-term recommendation specifically because of its cardiovascular benefit profile: it lowers your risk of stroke and heart attack, where retatrutide's added mechanism raises arrhythmia risk instead.

My Story

Three Years of Doing Everything Right — and Still Gaining Weight

I'd been slim my whole life, right up until I turned 40 — and then it happened almost overnight. All the little adjustments that used to work (tighten up the eating, drop the sugar, add a workout) just stopped doing anything. I was gaining three or four pounds a month for no reason I could find, and it kept happening even as I worked harder than ever.
I tried everything... Intermittent fasting. Lifting heavy. More cardio. Less cardio. High Fat, Moderate Protein. High Protein, Moderate Fat. Cutting Calories. Keto. Intuitive eating. You name it- if it was recommended as something that had helped women over 40 then I tried it.

At one point I paid $4,000 for a year-long, doctor-supervised program with a nurse dietitian reviewing every bite I ate — absolutely guaranteed to work for women over 40. I followed the guidelines to a "T".
I gained weight the entire time.

Then my eye doctor found something called drusen in my eye's capillaries and told me it was a sign of insulin resistance, essentially pre-diabetic. She recommended a hardcore carnivore diet. I tested my blood sugar 8 to 12 times a day for months and lived on beef, butter, and eggs. Everything I read said that if you stay in that extreme Keto state long enough, then the insulin resistance WILL eventually resolve.
It never did.
By that point I was three years and 40 pounds into this, and I didn't recognize myself in photos anymore.
